How often does a landlord gas safety certificate need renewing?+

Every 12 months, with the new certificate in place before the old one expires, not after. There's a buffer built in so a delayed appointment never pushes you past the date.

What actually happens if it lapses?+

Legally, it's a breach of the Gas Safety (Installation and Use) Regulations, which can mean a fine or prosecution. Practically, it can also invalidate landlord insurance, over something entirely preventable.
